General summary
Manages grant activities and healthcare initiatives to improve healthcare coordination for patients.
Major duties
- Oversees case management and care management services, including the clinical supervision of services and the supervision of case managers, case assistants and student interns.
- Schedules and facilitates patient interdisciplinary team meetings, periodic reviews and the creation of healthcare and support plans for patients.
- Meets with patients and healthcare teams as needed when patient is exhibiting symptoms of distress. Monitors service plans, follow-up on interventions and develop back-up crisis plans for services.
- Establish working relationships with staff at residential providers and community agencies. Serves as patient advocate and provide linkages to community services for improving healthcare coordination and self-determination.
- Develops clinical processes and protocols for healthcare coordination and clinical interventions. Provides crisis intervention for patients with immediate definable needs such as finances, shelter, food and medication control.
- Participates as member of the department’s management team. Oversees accreditation standards and contract compliance for services.
Minimum qualifications
Minimum qualifications based upon job documentation and industry best practices. Any current employees not meeting these qualifications will be grandfathered until they move to a different job.
Required education
- Master's degree in related field from an accredited institution.
Required experience
- Three years’ relevant experience.
- Experience providing case management services.
- Strong interpersonal, written and verbal communications skills.
Required other
- Fully licensed as a Licensed Masters Social Work (LMSW).
- Ability to work irregular shifts and extended hours, including evenings and weekends.

- Qualified Intellectual and Developmental Disability Professional (QIDP).
- Licensed as a social worker, psychologist, RN, LPN, counselor, or has a teaching certificate.
Physical requirements
Office or other indoor work with minimal physical demands such as occasionally lifting or moving materials less than 25 pounds.
Working conditions
Work is generally performed in a well-lit, temperature-controlled indoor environment with occasional exposure to the outdoors or any number of elements.
