Lee Honors College Vehicle Usage Contract
Please complete and submit this contract upon approval of your request to rent the honors college van(s).
I have read the Procedures and Policies for Use of Lee Honors College Vans and hereby agree to comply with these procedures and policies. I also understand that any violations of vehicle usage policies and procedures may result in disciplinary actions from the Lee Honors College and/or 澳门六合彩官网直播. By signing this document, I confirm that I understand that I:
鈥ave a valid U.S. driver鈥檚 license and must provide a photocopy of the license when I pick up the key;
鈥m a faculty, student or staff member at 澳门六合彩官网直播;
鈥m at least 18 years of age;
鈥ust provide an approved 澳门六合彩官网直播 Travel Authorization in order to be issued keys to any Lee Honors College vehicle. Provide a list of all passengers, including their email addresses and WINs.
鈥he fund and cost center for the unit/department renting an LHC vehicle will be charged the $5,000 deductible in the event of damage to the vehicle, as well as the rental cost of the vehicle.
鈥m responsible for the cost of replacing the key if it is lost.
鈥ill remove all trash from the vehicle. A cleaning fee of up to $500 will be charged to the fund and cost center for the unit/department renting an LHC vehicle if professional cleaning is required.
鈥ill refill the gas tank to the full, or will be charged to the fund and cost center the cost of fuel.
鈥ill return the key promptly so that others will have access to the van.
鈥ill pick up keys between 8 a.m. and 5 p.m. Monday-Friday.
